A recent report has revealed that a Fallright Lanyard Extension fitted with a tear away energy absorber has rolled out of its attachment point releasing the wearer of the harness which may have resulted in a fatal or serious injury incident.

FYI - these hooks are used around the world - in my training I single them out as a likely point of failure - here's a recent example! Note is from Loy Yang A power station. Please caution those you work with re this potential failure mechanism - not just rolling over onto the NRG absorber, but also pressing against another surface such as conveyor, structure, cherry picker bucket etc.
